How to Qualify for Assistance
You must meet the following criteria:
- If you are uninsured
- If you are under insured (which means that your yearly deductible is unattainable)
- Household income is less than $60,000
- If you are unable to monitor your blood sugar due to the expensive cost of glucose strips and are to use the meters/strips that we provide
- If your physician has recommended insulin pump therapy for you
If you meet all of the criteria listed above, please click the button below to fill out an Application Form. We will then determine if you are eligible.

CR3 Diabetes Association, Inc. incurs a cost/fee to have every insulin pump that we receive re-certified before we can provide it to an applicant. We are asking for your monetary support, whether it is a one time or monthly donation. Your support is important to people with diabetes who cannot afford the life-saving supplies that are required to manage this terrible disease.
